someone who loved a good joke, history and psychoanalysis.
Dr. Winer was an internationally known leader in psychoanalytic training, administration,
treatment and publication. He was editor of THE ANNUAL OF PSYCHOANALYSIS and
served as the Director of the Chicago Institute for Psychoanalysis from 1998 until 2004.
He had been Chair of the Committee on Ethics of the International Psycho-Analytical
Association and received its Award for Extremely Meritorious Service in 2005. He
contributed widely to psychiatric and psychoanalytic journals as well as serving on
several editorial boards. He was a past president of the American College of
Psychoanalysts, an honorary association of medical psychoanalysts.

Winer, J. A. & Ornstein, E. D. (2001) Titration in the Treatment of the More Troubled
Patient. Journal of the American Psychoanalytic Association 49:891-908
Abstract
This article defines and discusses a modification of technique recommended by the
authors in the psychoanalytic treatment of more troubled patients; a modification they
call titration. Titration is defined as a conscious decision by the analyst to increase or
decrease assistance (or gratification) gradually, in order to facilitate the analytic

process. The authors emphasize the complexity of decisions in treatment by focusing
on the decision-making processes that titration requires. Guidelines and a case vignette
are presented. The authors conclude by considering some of the politics involved in the
introduction of technique modifications, the salience of the titration concept, and
directions for further exploration.
